The beauty is that failure is predictable and avoidable. And when you see communication stop, that’s usually a sign that the work has stopped, that someone has gone into excuse mode (to rationalize their stopping of work or communication), and so forth.

Sometimes you see someone who has misrepresented their skill or intent, accidentally or willfully– and in this model, it’s easy to see and fix.

Culture and mission aside, mechanically, you can uncover why you’re failing, in the same way that a doctor can examine the x-ray, blood pressure, and vital.
